If you witnessed or experienced an incident in which you or someone else experienced harm at at Pas de Deux Photo, LLC event, please report that to us using this form.  If you are at an event, you can also use paper Incident Report forms, found at the Information Table.

Reports can be made anonymously.  If you do share your name, this allows PDD to follow up with any questions we may have.  Even if you do share your name, we will not reveal you are the reporter when speaking with the person about whom the report was made  The more details you provide, the more responsive PDD can be.
